Summary

We are seeking an experienced medical coder with a strong background in Hierarchical Condition Category (HCC) coding to support our healthcare reporting needs. The ideal candidate will have hands-on experience reviewing medical records, identifying accurate diagnoses, and assigning appropriate ICD-10-CM codes in compliance with CMS guidelines. This role will directly support our risk adjustment reporting and ensure coding accuracy for audit readiness. The ideal candidate will have a CPC certification and familiarity with Aprima HER.

This position will be required to work on-site in the greater Los Angeles area at a primary care office for 4-6 weeks.

Deliverables

• Review patient medical records and documentation to identify and validate HCC-related diagnoses.
• Assign accurate ICD-10-CM codes according to CMS risk adjustment and HCC coding guidelines.
• Ensure coding is compliant with regulatory and payer requirements.
• Prepare and review HCC coding reports to support risk adjustment submissions.
• Collaborate with our internal team to clarify documentation and resolve coding discrepancies.
